Permits can be issued by the Otselic Town Clerk for town residents only.  The application form must be completed by your physician.  Present the completed form to the Town Clerk. If it is determined to be a permanent disability the permit is issued for 5 years and can be renewed without a new application.  For a temporary disability, the permit is issued for no longer than 6 months but dependent on the diagnosis by the physician.  There is no charge for these permits.
